The purpose of this cluster randomized trial of 40 North Carolina hospitals (6000 patients) is to evaluate the comparative effectiveness of a patient-centered transitional care model versus usual care regarding functional outcomes of patients after a stroke, family caregiver strain, and hospital readmissions.
